Registration with the Clinic

Everystaff and student of the university is expected to register with clinic. Students are expected to register   at the time of registration and there after renew annually, while staff are expected to register only once at the time of documentation for employment. The procedure for registration for the student is as follows.

All newly admitted students are expected to be medically examined before registering in the university to ensure that they are fit enough to pursue the rigorous academic activities and also ensure that they do not constitute a danger to other students either at hall of residence or during lectures. There every student is expected to collect and complete the “A” part of the medical report form from the admissions office. The “B” part will be completed by the medical officers in the clinic. In addition to the basic requirements stated in the forms, a student may be asked (by the examining physician) to undergo additional laboratory investigations. A copy of the duly signed forms should be presented together with a signed form “02” and one passport size photograph to the medical records officer who in turn will issue a hand card with a hospital number on it. This is part of your identification in the university. Please keep it safe and present it whenever you require attention in the clinic. You may not be able to access any service in the clinic without your clinic hand card.

Every student (living both on and off the campus) of the university who becomes ill is expected to seek for medical attention in the university clinic. In the case of an emergency, where exigencies of time will not permit student to attend the clinic, he/she is expected to notify Director, Health Services as soon as is practically possible. The Director is duty bound to follow up and monitor the condition of the student.

 Medical certificates

Health Services Policy: Health Service physicians are available to assess students presenting with a medical concern, and provide treatment whenever possible. However, only students with a SERIOUS illness will be granted a certificate for deferral of an exam or other academic work.
Mild colds, non-acute diarrhea and/or vomiting, headaches, menstrual cramps, insomnia or other non-acute problems may benefit from medical advice and/or treatment, but may not warrant a certificate. Non-medical issues, such as a death in the family or sickness of a friend or other members of your family has nothing to do with health services department, that should be addressed through you staff adviser.

Please note: A Health Service physician can only provide a certificate if you were seen in the department while you were ill or injured, or for a past illness that is recorded on your medical record and affected your academic work. If you are ill and unable to attend an appointment with a health-care provider, but also unable to attend an exam; please notify your lecturer or faculty officer.
While our policy may seem strict, it is in place to ensure fairness. The physicians and nurses at the University Health Service are available and willing to help all students with their health care needs and to stay healthy throughout the academic year, including at exam time.
If for reasons of ill health, a student fail to perform his/her responsibility (lecture, assessment test, semester examination, practical class or any other activity) he/she is expected to collect a report from the registered practitioner that attended to him when he/she was sick, and present it to the Dean of his/her faculty who will then send it to the Director, Health Services for authentication. This gives the student adequate cover for the period of absence. Students outside Sokoto town or students taken on emergency to a hospital in Sokoto must similarly obtain a medical report from the physician that attended to him/her. It is in the best interest of students to get in touch, in the quickest possible time, with the university clinic and their respective faculties during the period of ill health and backup their cases with appropriate medical reports. The student affairs division can, on behalf of the student who gets in touch with it, communicate cases of ill health to the Director Health Services  or Dean of faculty pending their ability to resume school. The university does NOT honor medical reports from traditional healers including traditional bone setters.
